Billing Information for New Patients

The policy of our office is to collect any co-pay, co-insurance and non-covered fees at the time of service. If you have a specified co-pay with your insurance plan and are unable to pay that at the time of service, you will be charged for an additional rebilling fee of $25

Non-covered procedures may include, but are not limited to, refractions. A refraction may be performed to verify whether or not your vision can be improved with a new prescription or whether surgery is indicated for your medical condition. A refraction is considered routine by medicare as well as most other insurance plans, therefore, it is your responsibility to pay this fee when performed.

As a courtesy to our patients, we will bill all primary insurance coverage if we are a participating provider. If we are not a contracted provider, payment will be expected at the time of service.
